Leap Sweepstakes Official Rules
NO PURCHASE NECESSARY TO ENTER OR WIN. A PURCHASE OR PAYMENT OF ANY KIND WILL NOT INCREASE YOUR CHANCES OF WINNING.
1.Sponsor
This Sweepstakes (“Sweepstakes”) is sponsored by Leap Health, Inc., with a principal place of business at 1 Rockefeller Plaza, 2nd Floor, New York, New York 10020 (“Sponsor”). 2.Eligibility
The Sweepstakes is open to legal residents of the United States (except residents of New York, Florida, and Rhode Island, who are not eligible to participate) who are at least eighteen (18) years of age at the time of entry, who are attendees of the 2026 Employee Health Care Conference Chicago held on September 24, 2026 through September 26, 2026 in Chicago, Illinois (“Event”), and who are employees of current or potential clients of Leap Health MSO, LLC. Officers, directors, and employees of Sponsor, and their respective parent companies, subsidiaries, affiliates, advertising and promotion agencies, and the members of their immediate families (spouse, parents, siblings, and children) and household members of each such individual, whether or not related, are not eligible to participate. Void in New York, Florida, Rhode Island, and where otherwise prohibited or restricted by law.
3.Sweepstakes Period
The Sweepstakes begins on August 26, 2026 at 7:15 AM Central Time and ends on September 26, 2026 at 12:30 PM Central Time (the “Sweepstakes Period”).
4.How to Enter
NO PURCHASE NECESSARY. NO PURCHASE WILL IMPROVE CHANCES OF WINNING.
- Method 1: In-Person Entry (at the Event). Visit the Sponsor’s booth or designated entry area at the Event during the Sweepstakes Period. Complete and submit a digital entry form with your full name and email address.
- Method 2: Register for the Benefit Blend co-hosted by Leap on September 24, 2026 via invitation e-mail.
- Method 3: Register for the Event using Leap’s free registration code via invitation e-mail.
- Method 4: Free Alternate Method of Entry (AMOE). Handprint your full name, email address, phone number, state of residence, and mailing address on a 3” x 5” card and mail it in a stamped envelope to: Leap Sweepstakes, 1 Rockefeller Plaza, 2nd Floor, New York, NY 10020. Mail-in entries must be postmarked during the Sweepstakes Period and received by September 26, 2026. Limit one (1) mail-in entry per person per stamped envelope.
All methods of entry have equal dignity. Entries submitted via the AMOE will receive the same chances of winning as entries submitted in person. Limit one (1) entry per person, regardless of method of entry. Entries generated by script, macro, or other automated means are void. All entries become the property of Sponsor and will not be returned. Entries received from residents of New York, Florida, or Rhode Island will be disqualified.
5.Winner Selection and Odds
On or about September 26, 2026, one (1) winner will be selected in a random drawing from among all eligible entries received during the Sweepstakes Period. The drawing will be conducted by Leap.
6.Prize
One (1) Grand Prize: A Louis Vuitton Neverfull MM tote bag. Approximate Retail Value (“ARV”): $2,170.00. The odds of winning depend on the total number of eligible entries received during the Sweepstakes Period.
The prize is awarded "as is" with no warranty or guarantee of any kind, either express or implied. No cash equivalent, substitution, or transfer of prize is permitted, except that Sponsor reserves the right, in its sole discretion, to substitute a prize or equal or greater value if the advertised prize becomes unavailable for any reason.
The winner is solely responsible for all federal, state, and local taxes, including income taxes, and any other costs and expenses associated with prize acceptance and use that are not specified herein.
7.Winner Notification and Prize Claim
The potential winner will be notified by email and/or phone within five (5) business days of the drawing. The potential winner must respond within two (2) calendar days of notification, or the prize may be forfeited, and an alternate winner may be selected.
As a condition of receiving the prize, the potential winner must execute and return within ten (10) calendar days: (a) an Affidavit of Eligibility and Release of Liability which will include verification that the winner is not a resident of New York, Florida, or Rhode Island; and (b) where legally permissible, a Publicity Release granting the Sponsor the right to use the winner’s name, likeness, city, and state of residence, photograph, and statements regarding the Sweepstakes for promotional purposes in any and all media, without additional compensation, except where prohibited by law. The Publicity Release applies solely to Sponsor’s use of the winner’s information in connection with this Sweepstakes and does not constitute consent to receive marketing communications from Sponsor (see Section 9). The winner must also provide a completed IRS Form W-9 (or equivalent) prior to prize fulfillment. Because the ARV of the prize exceeds $2,000, Sponsor will issue an IRS Form 1099 to the winner reflecting the value of the prize.
If a potential winner is found to be ineligible, including if the potential winner is a resident of New York, Florida, or Rhode Island, or if a potential winner does not comply with these Official Rules, or if prize notification is returned as undeliverable, the prize will be forfeited and, at Sponsor's sole discretion, an alternate winner may be selected.
8.Privacy and Data Collection
By entering the Sweepstakes, each entrant acknowledges and agrees that Sponsor will collect personal information (full name, email address, phone number, state of residence, and mailing address) for the purpose of administering the Sweepstakes and fulfilling the prize. This information will be processed in accordance with Sponsor's Privacy Policy, available at https://www.leaphealth.com/privacy-policy, and applicable law.
Entrants may also designate an authorized agent to submit a request on their behalf, subject to verification procedures described in Sponsor’s Privacy Policy.
Entrants are not required to opt in to marketing communications as a condition of entry.
9.Marketing Consent
Entry into the Sweepstakes does not constitute consent to receive marketing communications from Sponsor and does not constitute a publicity release (see Section 7 for the limited publicity rights granted by the winner only). If Sponsor wishes to send marketing emails, text messages, or other communications to entrants, it will obtain separate, affirmative opt-in consent at the time of entry, clearly distinguishable from the Sweepstakes entry itself. Any such communications will comply with applicable federal and state laws, including the CAN-SPAM Act and the Telephone Consumer Protection Act (TCPA).
10.Release and Limitation of Liability
By entering, each entrant agrees to release and hold harmless Sponsor, and its respective parent companies, subsidiaries, affiliates, advertising and promotion agencies, and their respective officers, directors, employees, agents, and representatives (collectively, the "Released Parties") from and against any and all claims, actions, damages, losses, costs, expenses, or liability (including reasonable attorneys' fees) arising out of or in connection with: (a) participation in the Sweepstakes; (b) acceptance, possession, use, or misuse of a prize; or (c) any claims based on publicity rights, defamation, or invasion of privacy.
The Released Parties are not responsible for: (i) lost, late, incomplete, illegible, misdirected, stolen, or undelivered entries; (ii) any technical malfunctions, errors, or failures; (iii) unauthorized human intervention in any part of the entry process or the Sweepstakes; or (iv) any injury, damage, or loss arising from participation in the Sweepstakes or acceptance or use of a prize.
11.Right to Cancel or Modify
Sponsor reserves the right, in its sole discretion, to cancel, suspend, or modify the Sweepstakes if any fraud, virus, bug, or other cause beyond Sponsor's control corrupts or affects the administration, security, fairness, or proper conduct of the Sweepstakes.
12.Disputes and Governing Law
These Official Rules and the Sweepstakes are governed by the laws of the State of Illinois, without regard to conflicts of law principles.
13.Winners List
For the name of the winner, available after September 26, 2026, send a self-addressed, stamped envelope to: Leap Health Sweepstakes Rules/Winner Request at 1 Rockefeller Plaza, 2nd Floor, New York, NY 10020. 14.Advertising Disclosures
All advertising and promotional materials for this Sweepstakes must include the following disclosures in a clear and conspicuous manner:
- "NO PURCHASE NECESSARY."
- "A PURCHASE WILL NOT IMPROVE YOUR CHANCES OF WINNING."
- The eligibility requirements (age and geographic restrictions), including that residents of New York, Florida, and Rhode Island are not eligible.
- The start and end dates of the Sweepstakes Period.
- The name of the Sponsor.
- Instructions on how to obtain the complete Official Rules free of charge.
