At Leap Health, our mission is to make infusion therapy safe, convenient, and transparent. These Terms of Service explain how our coordination services work, your responsibilities, and what you can expect from us.

Leap Infusion Services

This document outlines the terms and conditions for the infusion services coordinated by Leap Health (“Leap” or “we”). 

1. Our role

Leap coordinates infusion services through partnerships with licensed nursing agencies, specialty pharmacies, and other licensed practitioners. Our goal is for you to receive high quality, convenient care. We do not directly provide nursing services or medications. We arrange for these services to be delivered in your home or to other convenient locations. The medications are prescribed by your physician and dispensed according to that prescription. 

Leap does not provide medical care, medical advice, diagnosis, or treatment. The licensed professionals who provide your care are employed by third parties, not Leap. 

2. Your responsibilities

You are responsible for:

  • Providing accurate and complete medical information to Leap and the assigned nurse.
  • Being present and ready for your scheduled appointments.
  • Notifying Leap of any changes to your health or medication, or if you need to reschedule or cancel an appointment.

3. Data privacy and sharing

To provide these services, Leap needs to share your personal and health information with the following parties:

  • The specialty pharmacy that dispenses your medication.
  • The nursing agency and the specific nurse administering the infusion.
  • Your prescribing physician.
  • Your insurance company for billing purposes.
  • Other parties as required by law.

Your privacy is important to us, and we are very careful about how we use data that identifies you and with whom we share it. We are committed to handling your information in accordance with applicable federal and state laws, including HIPAA (Health Insurance Portability and Accountability Act). 

4. Billing and financial responsibility

Leap will coordinate with your insurance provider. You are responsible for any co-pays, deductibles, or other out-of-pocket costs not covered by your insurance plan. For many patients, Leap is available at no additional cost to you.

5. Limitation of liability

Leap’s liability is limited to the coordination of services. Leap is not liable for the actions or omissions of partner nursing agencies, specialty pharmacies, or other licensed practitioners, beyond reasonable coordination efforts.

6. Right to terminate services 

You have the right to stop receiving these services at any time. To do so, you must notify Leap in writing. This revocation will not affect any actions taken by Leap or its partners prior to the date of your notification.
