Your patient journey all starts when a Leap Care Guide reaches out to coordinate your infusion care. Your Care Guides are here as your single point of contact in coordinating care and ensuring that any questions are answered and special requests are recorded.
Leap’s team works directly with patients to determine eligibility for financial assistance. Many drug manufacturers offer programs for patients to offset high out-of-pocket costs that can occur when a patient is prescribed specialty medications. Leap’s team ensures patients don’t miss out on opportunities to receive financial assistance from these various programs.
Your Care Guide will gather dates and times that work with your busy schedule and coordinate care with Leap’s national network of in-home infusion nurses.
Once care has been scheduled, your Care Guide will work with one of Leap’s accredited specialty pharmacies to send you your prescription. Specific handling and storage requirements will also be included.
On the day of your appointment, your Leap nurse will arrive at your home and administer your treatment wherever is most comfortable for you. Your Leap nurse will also answer any questions you have about your treatments and be able to make adjustments as needed.
For follow-up visits, patients work directly with their dedicated Care Guide. If questions arise, medical or otherwise, patients have access 24/7 to clinical support from Leap as well.
Leap facilitates the sharing of visit records and patient notes back to the patient's prescribing physician - ensuring open lines of communication and enabling our clinical team the opportunity to work with prescribing physicians to optimize care.
Leap works directly with patients to determine eligibility and apply for further financial assistance for their medical treatments. Many manufacturers offer financial assistance programs to patients who meet certain income requirements.
